CMR–based Modelling of the Left Ventricle: a Tool for Functional Assessment and Surgical Decision Support
نویسندگان
چکیده
The most challenging objective of the clinical decision making in the management of patients with myocardial dysfunction is to choose the best option for improving myocardial function and clinical outcome. The present work aims at developing patient-specific computational models to assist cardiologists and cardiac surgeons in defining the severity and extent of disease in patients with left ventricular (LV) dysfunction, with or without mitral regurgitation, and in supporting clinical decision making and planning of the optimal treatment for left ventricle-valve repair. Two software were developed: the Functional Assessment Tool (FAT) provides cardiologists with the automatic extraction of quantitative MRI-derived parameters about LV performance; the Functional Predictive Tool (FPT) allows cardiac surgeons for easy and fast simulations of post-operative scenarios for different surgical choices. FAT validation, focused on LV dimension and function indices, demonstrated a good ability in detecting endoand epicardium and provided results comparable to the gold standard. Preliminary simulations of surgical procedures using FTP allowed to calculated segmental circumferential and radial strains with sufficient accuracy. Future developments will include the tuning of simulation parameters used accordingly with the feedback of clinicians and the effects of changes in peripheral resistances and heart rate, typically associated to post-operative scenarios.

The stochastic active contour scheme (STACS) is a well-known and frequently-used approach for segmentation of the endocardium boundary in cardiac magnetic resonance (CMR) images. However, it suffers significant difficulties with image inhomogeneity due to using a region-based term based on the global Gaussian probability density functions of the innerouter regions of the active ...
متن کاملLeft ventricular modelling: a quantitative functional assessment tool based on cardiac magnetic resonance imaging.
We present the development and testing of a semi-automated tool to support the diagnosis of left ventricle (LV) dysfunctions from cardiac magnetic resonance (CMR). CMR short-axis images of the LVs were obtained in 15 patients and processed to detect endocardial and epicardial contours and compute volume, mass and regional wall motion (WM). Results were compared with those obtained from manual t...
متن کاملDevelopment of decision support tool for municipal solid waste management system in Iran based on life cycle assessment approach
Background and Objective: Various aspects including the environmental burdens, social and economic consequences of the waste management(WM) scenarios must be considered to come up with a comprehensive WM plan. Life Cycle Assessment (LCA) approach is a systematic method to quantify the environmental burdens of each WM scenario. Materials and Methods: This study used an LCA approach to develop a...

Baseline data represent one of the important stages of Environmental Impact Assessment (EIA) procedure that describes the existing environment of the study area and surrounding areas in enough detail to allow the environmental impacts of the proposed area to be accurately and adequately assessed, and future changes and effects can be measured. Baseline data may be inaccurate, difficult to obtai...
متن کاملReview of the role of cardiovascular magnetic resonance in congenital heart disease, with a focus on right ventricle assessment.
Adult patients with congenital heart disease (ACHD) represent a growing population due to progress in management. Surgical procedures generally fall short of restoring entirely normal anatomical and functional relations. Further procedures can be needed and lifelong follow-up is required. The right ventricle (RV) plays an important role in congenital heart disease and cardiac magnetic resonance...
